REFUND POLICY

All tickets are non-refundable, but they ARE transferrable.

We do not apply discounts retroactively.

Once you have registered, your fees will not be adjusted downward based on any sponsorship, promo code, discount or any other reason. Virtual tickets are only valid for one user.

Unused Group Tickets, e-Tickets, logins, etc., are not refundable. Additionally, unused Group Tickets, e-Tickets, logins, etc., are not transferrable to future events or redeemable for cash or applicable to apply to credit for other online, live, in person or future events of any kind.

Concierge Medicine Today does not and is not obligated to provide a credit voucher to registered attendee(s) who do not attend the event they registered for or said they did not receive the multiple reminder(s) for. We recommend attendees routinely check their Junk Mail folder or SPAM if they feel they cannot find their login or URLs to attend the event.

DO I GET A REFUND IF I AM UNABLE TO LOG-IN?

See our refund policy below. The simple answer is NO.

We provide 6-weeks of on-demand access via our online Community at www.ForDoctorsForum.org to our conference materials, speakers and presentations. So whether you are unable to log-in at a certain time or have issues logging in … all tickets are non-refundable, but they ARE transferrable. Unused tickets have no cash value and will not be refunded.

How Attendees and or New or Current Members/Attendees Join

No matter what your privacy settings are, all members are required to sign up and join your Mighty Network at “Concierge Medicine Forum” by using either the options they see to log in using Facebook or LinkedIn, or by entering their first name, last name, and email address, then setting up a password they can use going forward to log in.

What information in my profile is accessible to other attendees?

TRANSFER TICKET POLICY

Event ticket purchases are transferrable if you are unable to attend but not refundable. If you’ve purchased a ticket and are not able to attend for some reason, we encourage you to watch all of these on-demand sessions and presentations at our on-demand FOR DOCTORS Virtual Vault until December 31st, 2020 at midnight.

We are not responsible for and will not refund individual or corporate attendees for any expenses related to your planned conference attendance in-person or digital. If an event or presenter gets postponed or rescheduled to a future date or is unable to attend, the tickets will automatically transfer to that future event date or an event credit for use towards another future event will be offered upon request.

INCLUDED WITH EACH TICKET

Each ticket buys a login and password for one person to access the online 2020 Concierge Medicine Forum. Dependent upon what level of ticket you purchased will determine the access to content you are able to acquire. You will receive an email to the address you used at checkout with login and permissions at least one week prior to the online event. At Mighty Networks, which is the software/technology platform we use at Concierge Medicine Today, LLC., to host our events, live chats, conferences, courses, etc., MightyNetworks takes privacy and data security very seriously, and they have done extensive work to ensure that oour Mighty Network is GDPR compliant and your data (and your members’ data) is stored securely. 

Our Privacy Promise (Mighty Networks)

As the Host of a Mighty Network, you have the ability to choose the privacy settings you want to enable for your community. Communities that are private, secret, or paid are completely locked down and non-members (including search engines) cannot see the content, memberships, and conversations inside your Mighty Network.

If you choose to make your Mighty Network private or secret, only members with an invite will be able to join. You as the Host of your community also have the power to remove a member or ban them at any point.

Hosts have complete control over the content they add and the information they share within their own Mighty Network. We do not censor content or remove content, provided you and your members are participating within our Terms of Use.

Mighty Networks and Your Members

Mighty Networks will never email your members, and your members do not receive any communications (marketing or otherwise) from Mighty Networks directly, outside of the notifications they opt into that come from your community when they join.

Occasionally, you as a Host may receive communications from our team about feature updates and announcements, but those emails will only go out to folks who are Hosts and creators of their own Mighty Networks. Only you as the Host (or anyone else on your team who is listed as a Host) should be getting those emails.

Mighty Networks takes data security very seriously, and our engineers complete regular API reviews and code reviews to address security issues upfront. In addition, a full suite of tests runs in our Continuous Integration (CI) systems to verify the security measures that have been put in place continue to function on an ongoing basis.

Our core team includes engineers with security training from Microsoft and Symantec (among others), experienced in threat modeling and security reviews. They bring this experience to bear when reviewing the work of other engineers and making security decisions.

NOTICE of FILMING and PHOTOGRAPHY:
When you enter a Concierge Medicine Today (CMT) FORUM or event, room, breakout session, property outside/parking lot/picnic tables, workshop, reception, party, general session, hallway, lobby, etc., or program, you enter an area where photography, audio, and video recording may occur. By entering the event premises, wherever the event is hosted, you consent to interview(s), photography, audio recording, video recording and its/their release, publication, exhibition, or reproduction to be used for news, web casts, promotional purposes, telecasts, advertising, inclusion on websites, social media, or any other purpose by CMT and its affiliates and representatives. Images, photos and/or videos may be used to promote similar CMT events in the future, highlight the event and exhibit the capabilities of CMT. You release CMT, its officers and employees, and each and all persons involved from any liability connected with the taking, recording, digitizing, or publication and use of interviews, photographs, computer images, video and/or or sound recordings.

Photography, Recording, and Videotaping. You may not record audio or video of physical or virtual sessions at any event, session, breakout or gathering, whether informal or formal while on site at the event. 

By entering the event premises, you waive all rights you may have to any claims for payment or royalties in connection with any use, exhibition, streaming, web casting,televising, or other publication of these materials, regardless of the purpose or sponsoring of such use, exhibiting, broadcasting, web casting, or other publication irrespective of whether a fee for admission or sponsorship is charged. You also waive any right to inspect or approve any photo, video, or audio recording taken by CMT or the person or entity designated to do so by CMT.

By purchasing a ticket [or otherwise accompanying/entering or being in the vicinity of the event] you have been fully informed of your consent, waiver of liability, and release before entering the event. I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I AGREE TO IT OF MY OWN FREE WILL.

CONSENT FORM, WAIVER, Acknowledgements and RELEASE of LIABILITY

I furthermore acknowledge that CMT and their employees, contract workers, volunteers, directors, officers, representatives, and agents are NOT responsible for the errors, omissions, acts, or failures to act of any party or entity conducting a specific event or activity on behalf of CMT. I acknowledge that this activity or event may involve physical activity, and may carry with it the potential for death, serious injury, and property loss. I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during this activity or event. I understand that at this event or related activities, I may be photographed. I agree to allow my photo, video, or film likeness to be used for any purpose by the event holders, producers, sponsors, organizers, and assigns. The accident waiver and release of liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.

By participating, registering, speaking, sponsoring, and/or attending and being in or outside the building near the event, etc., you are acknowledging that an inherent risk of exposure to COVID-19 or other health risk(s) exists in any public place where people are present. By attending the CMT FORUM you and any guests voluntarily assume all risks related to exposure to COVID-19 and any other health risk(s) and agree not to hold Concierge Medicine Today, LLC., or any of their affiliates, directors, officers, employees, agents, contractors, or volunteers liable for any illness or injury.

If you do not feel well or are experiencing any symptoms of COVID-19, please do not attend.

Upon entering the event venue, attendees may be asked to scan a QR code that will take them to a questionnaire to mark their attendance, answer screening questions, and have their temperatures taken. Temperatures may be taken by a masked volunteer or employee or representative. If an in-person event attendee has a temperature of 100.4 or greater they will NOT be permitted in the event venue.

There will be multiple sanitation stations located throughout the venue.
Please follow all social distancing guidelines including, remaining 6 ft. apart and refrain from shaking hands.

Seating will be spaced out to comply with the social distancing guidelines.
We ask you to please wear a mask out of respect for guests and team members. Masks will be provided for your use but are not mandatory.

“Any person entering the premises waives all civil liability against this premises owner and operator for any injuries caused by the inherent risk associated with contracting COVID-19 at public gatherings, except for gross negligence, willful and wanton misconduct, reckless infliction of harm, or intentional infliction of harm, by the individual or entity of the premises.”

I understand the guidelines which CMT has set in place, and I agree to uphold these guidelines to help keep myself and other attendees safe. If I choose not to uphold these guidelines, I understand that I could be asked to leave the event and will not receive a refund.

By registering, attending, participating, speaking, sponsoring, etc., I acknowledge fully the risk(s) I am taking and yes, I have read and fully understand the risks associated with attending in-person events. And, I agree to not hold CMT and its employees, represenatives or the Forsyth Conference Center and its employees liable for any illness or harm that could arise from participating in this event in-person.
