What is included with my ticket?
We Have Two Types of Tickets …
– 3 Day All Access Pass or
– 2 Day Conference Pass
Included in your ticket is admission to the 2-day conference (unless you purchased/registered for a 3-day pass — accessing one of the Lead-In Workshops as well):
Two Day Pass Ticket/Registration Includes: Continental Breakfast on Day One; lunch on Day One; Friday (Day One) Cocktail Reception; Continental Breakfast on Day Two; and all coffee/beverage service and snacks available throughout the event.
Three Day All Access Pass Includes: One Lunch (Per Registered Person included) during Each Pre-Conference Workshop; and all coffee/beverage service and snacks available throughout the workshop; Continental Breakfast on Day One; lunch on Day One; Friday (Day One) Cocktail Reception; Continental Breakfast on Day Two; and all coffee/beverage service and snacks available throughout the event.

WHEN DO I REGISTER FOR BREAKOUTS?
We recommend that you select your breakouts right away—while you are registering for the event. You can choose from between any of our breakouts that represent a variety of ministry areas. Pick what is best for your role. But hurry—the most popular ones will fill up quickly. Please email the CMT FORUM team at email@example.com or call (770) 455-1650 ext 111 if you have any questions.
WHY DID A BREAKOUT I WANT TO ATTEND FILL UP?
Our breakout rooms vary in size and seating capacity. We have to cap the breakouts according to their assigned rooms. We cannot guarantee space in a specific breakout, so please register for your first-choice breakouts as soon as possible.
CAN I CHANGE MY BREAKOUT SELECTIONS?
Yes, you can change your breakout selections at any time while the registration window is open. Some of the breakouts will fill up quickly, so we encourage you to make your final selections as soon as possible.
WILL YOU HAVE TRANSLATORS?
At this time, we do not offer translators or translation services. Due to the disruption and discomfort this may create for the majority of our guests, no audible translations will be permitted in any breakouts or main sessions.
WILL YOU RECORD or live stream THE conference?
At this time, we do not offer recording services or live streaming services. We have found over the years that very few attendees truly go back and listen to them. It’s also pretty costly and due to the disruption and distraction this can create for the majority of our attendees and guests, no audio or video streaming or recording will be permitted in any workshops, breakouts or main sessions. We will however provide ALL (permitted) presenter “Slide Decks” to our registered guests after the event is over.
WILL THERE BE COFFEE ONSITE?
Yes! Of course and a lot of it! There will be free, delicious coffee available throughout the conference.
Does Our Facility Have A Recycling Program?
We don’t have control over this but that’s a great question, and an important one. If this is important to you and critical as to whether or not you attend a conference, please feel free to call the Facilities Director/Event Planner at the Forsyth Conference Center at (678) 341-6633.
ARE CHILDREN ALLOWED?
We are unable to accommodate children at the conference, and they will not be allowed in the main sessions or the breakouts. Please make alternate arrangements for any childcare needs.
WHAT DO I NEED AT CHECK-IN?
Please have your Eventbrite Ticket/Payment Confirmation email handy at check-in. Thank you!
WILL I RECEIVE ANYTHING IN THE MAIL BEFORE CHECK-IN? No.
We will distribute all conference materials at check-in or during the event.
Why Don’t You Hand Out Slide Decks of Presenters BEFORE The Event?
Good question. We’ve been doing this for a very long time. We understand the needs our attendees and our Speakers. We routinely have over 50% of our Speakers so excited about their presentation at our FORUM that they often make last minute adjustments to their presentations to you at the last minute. Therefore, it’s impossible to have all those changes inserted, printed and distributed to our guests before you arrive.
We will however, distribute all conference materials at check-in or during the event. PLUS, we make ALL of our “Permitted” Slide Decks available within 10-business days AFTER the Conference to ALL of our Attendees. Not too mention, if you look at the reverse side of your name badge/printed lanyard, you’ll find instructions on how and where to download these “Slide Decks” which have ALL the changes we talked about inserted into them that our faculty, Physicians and presenters may have made along the way!
IS THERE A PARKING LOT?
Yes. Lanier @ Forsyth Conference Center has plenty of free parking spaces available.
WHAT IS THE CLOSEST AIRPORT?
We recommend Hartsfield-Jackson Atlanta International Airport.
A SPEAKER IS MISSING FROM THE ORIGINAL LINEUP. WHAT HAPPENED?
Emergencies happen and sometimes speakers have to cancel due to unexpected circumstances. It happens on seldom occasions, not often … but we roll with the punches. Please note that we are unable to offer refunds due to speaker cancellations.
I’M INTERESTED IN BEING A VENDOR OR SPONSOR.

2020 CMT FORUM TERMS & CONDITIONS
TERMS AND CONDITIONS:
By registering for an Concierge Medicine Today, LLC. (“CMT”) event, you agree to the following set of terms and conditions listed below:
- CMF event ticket purchases are transferrable if you are unable to attend. In the case an event has to be cancelled with no plan to reschedule, tickets purchased will be refunded. If an event gets postponed, moved to a virtual platform or rescheduled to a future date, the tickets will automatically transfer to that future event date.
- Credit vouchers are valid for 18 months from date of issue. Credit vouchers are valid toward one (1) Concierge Medicine Today event of equal or lesser value. If the full amount of said voucher is not used at time of registration, any remaining balance is not applicable now or in the future. Once a credit voucher has been applied toward a future event, changes cannot be made. In the event of cancellation on the attendees’ behalf, the credit voucher will no longer be valid.
- Concierge Medicine Today does not and is not obligated to provide a credit voucher to registered attendee(s) who do not attend the event they registered for unless written notice of intent to cancel is received and confirmed prior to the commencement of the event.
- Concierge Medicine Today reserves the right to cancel any event it deems necessary and will not be responsible for airfare‚ hotel or any other expenses incurred by registrants.
- Concierge Medicine Today’s liability is limited to the conference registration fee in the event of a cancellation and does not include changes in program date‚ content‚ speakers and/or venue.

Make checks payable to “Concierge Medicine Today” and write FORUM on your check. You may also use Visa, MasterCard, Discover or American Express. Payments must be received in full by the conference date. Any discount applied cannot be combined with any other offer and must be paid in full at the time of order. Parties must be employed by the same organization and register simultaneously to realize group discount pricing options.
NOTICE OF FILMING AND PHOTOGRAPHY
NOTICE of FILMING and PHOTOGRAPHY:
When you enter a Concierge Medicine Today (CMT) FORUM or event, room, breakout session, property outside/parking lot/picnic tables, workshop, reception, party, general session, hallway, lobby, etc., or program, you enter an area where photography, audio, and video recording may occur. By entering the event premises, wherever the event is hosted, you consent to interview(s), photography, audio recording, video recording and its/their release, publication, exhibition, or reproduction to be used for news, web casts, promotional purposes, telecasts, advertising, inclusion on websites, social media, or any other purpose by CMT and its affiliates and representatives. Images, photos and/or videos may be used to promote similar CMT events in the future, highlight the event and exhibit the capabilities of CMT. You release CMT, its officers and employees, and each and all persons involved from any liability connected with the taking, recording, digitizing, or publication and use of interviews, photographs, computer images, video and/or or sound recordings.
By entering the event premises, you waive all rights you may have to any claims for payment or royalties in connection with any use, exhibition, streaming, web casting,televising, or other publication of these materials, regardless of the purpose or sponsoring of such use, exhibiting, broadcasting, web casting, or other publication irrespective of whether a fee for admission or sponsorship is charged. You also waive any right to inspect or approve any photo, video, or audio recording taken by CMT or the person or entity designated to do so by CMT.
By purchasing a ticket [or otherwise accompanying/entering or being in the vicinity of the event] you have been fully informed of your consent, waiver of liability, and release before entering the event. I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I AGREE TO IT OF MY OWN FREE WILL.
CONSENT FORM, WAIVER and RELEASE of LIABILITY
RELEASE of LIABILITY
I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING AND/OR ATTENDING THIS ACTIVITY OR EVENT, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault. I certify that I am physically fit, have sufficiently prepared for participation in the activity or event, and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in this activity or event. I acknowledge that this Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity or event in which I may participate, and that it will govern my actions and responsibilities at said activity or event. In consideration of me wanting to participate in this event, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows: (A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage,property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this event, THE FOLLOWING ENTITIES OR PERSONS: Concierge Medicine Today (CMT) their directors, officers, employees, volunteers, representatives, and agents, the activity or event holders, activity or event sponsors, activity or event volunteers; Furthermore, I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE Concierge Medicine Today (CMT) the entities or persons mentioned in this paragraph from any and all liabilities or claims made as a result of participation in this activity or event, whether caused by the negligence of release or otherwise. I furthermore acknowledge that CMT and their employees, contract workers, volunteers, directors, officers, representatives, and agents are NOT responsible for the errors, omissions, acts, or failures to act of any party or entity conducting a specific event or activity on behalf of CMT. I acknowledge that this activity or event may involve physical activity, and may carry with it the potential for death, serious injury, and property loss. I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during this activity or event. I understand that at this event or related activities, I may be photographed. I agree to allow my photo, video, or film likeness to be used for any purpose by the event holders, producers, sponsors, organizers, and assigns. The accident waiver and release of liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.
