  • CODE OF CONDUCT FOR ALL CMF Participants
    Read T&Cs

    CODE OF CONDUCT. You must adhere to CMT’s Code of Conduct. CMT will enforce this code and expects cooperation from all Conference participants (e.g., attendees, guests, spouse, speakers, media, agents, employees, co-workers, friends, sponsors, volunteers, etc.) to help promote a safe and enjoyable Conference experience for everyone.

    Be supportive. Encourage and support your fellow members. No one here is looking for your criticism, cynicism, advice, or judgement.

    Share generously. Your stories and experiences may be exactly what another member needs to hear today to solve a problem or seize an opportunity.

    Be constructive. We’re here to push each other forward and lift each other up. Find ways to help each other think bigger, reframe challenges, and stay curious.

    Don’t spam, canvas, over sell, blatantly promote, or troll. This event/conference/community exists to help others learn. Give more than you take to this event. Self-promotion, spam and irrelevant, unethical guidance is strictly prohibited and is not allowed. It’s not a place to spam, canvas, solicit or bully anyone else.

    Respect Everyone’s Opinion and Privacy. Being part of this event and/or discussions and group(s) whether online or live and in-person, requires mutual trust. Authentic, expressive discussions make events like this (online and offline) great but may also be sensitive and private. What’s shared in the private should stay in the private setting but do however understand this is a public forum and this is to be treated as a public square, therefore, please refrain from sharing private and/or proprietary information you would not desire to be public knowledge in and/or at this event.

    I will have an open mind and a positive attitude. While at Conference, CMT expects you, any attendees, sponsors, exhibitors, and their representatives to foster a safe and positive experience for everyone. Unacceptable behavior which may or may not be limited to: rude, offensive or disruptive speech; offensive or rude social media post(s); heckling; demonstrating or selling a product as an attendee and not an authorized and approved by CMT, Conference exhibitor; defaming speech or defaming social media use; offensive photos; mischaracterizations of any person(s) in attendance; misinterpretation of information;  inappropriate photo(s) taken; misuse or damage to property of any kind, including Venue or any individuals personal belongings; unlawful activity; rude behavior; accused of sharing offensive opinion(s) or rude social media posts while at Conference; hate speech; discriminatory or offensive gestures; etc.

    Unlawful Activity. We reserve the right to investigate complaints or reported violations of this Agreement and to take any action we deem appropriate, including, but not limited to, reporting any suspected unlawful activity to law enforcement officials, regulators, or other third parties and disclosing any information necessary or appropriate to such persons or entities relating to your profile, email addresses, usage history, posted materials, IP addresses, and traffic information.

    Unprofessional Character. CMT reserves the right to remove any person and their party, company, parties, sponsor, exhibitor or individual(s) at its sole discretion for any reason it deems necessary, and you understand entry may be banned by CMT with no reason, legal claim, recourse, refund or remuneration. Under no circumstances is derogatory, defaming comments about individuals, companies, products, or services, or otherwise to be tolerated. Hard selling, canvasing, soliciting, expressions of rude behavior(s), profanity, or any other similar behavior as deemed by CMT, it’s respective representatives and/or another Conference attendee, venue staff or exhibitor or sponsor. Please make sure everyone feels safe. Racial phrases, bullying of any kind, hate speech and/or political character assassinations and discrimination of any sort are strictly prohibited, not allowed, and will not be tolerated. No degrading comments about things like race, religion, culture, politics, sexual orientation, business choices, business models, gender or identity will not be tolerated. While all Political and religious views should be accepted and tolerated, please bear in mind this is a public and business event. Attendees typically use this time to learn about business and leadership, hear medical education information, learn about healthcare products and services and Conference(s) such as this are understood to be a public forum whereby it is an escape from all the chaos on the news channels, distractions at work/home, and a social media timeline.

    Impeding Others Ability To participate, promote, advertise, network, learn, discuss, co-exist, or operate in this Conference and/or environment. This is understood to be a public forum in which Conference attendees, speakers, exhibitors, and sponsors desire to learn and network. By becoming a participant (e.g., sponsor; attendee or any other type of paid or unpaid or invited participant), you consent to respect other people, opinions, views, organizations, and their representative(s), and will not in any way impede their ability to participate, promote, advertise, network, learn, discuss, co-exist or operate in this Conference and/or environment.

    Reporting Code of Conduct Violation(s). Any breach of these Registration Terms and Conditions, observations, SPAM and/or harassment during Conference should be immediately reported to CMT and a Venue staff person immediately. If such a violation is brought to CMT or Venue representative(s) attention, even suspected, arises, witnessed second-hand, observed and/or heard from, suspected or reported by a third party, or any of the like, all individual(s) in their party, any person(s), any exhibitor(s), any speaker(s), any sponsor(s), etc., will be asked to leave and potentially the police involved. Attendance and any future inclusion or participation in the Conference and future CMT activities may also be immediately terminated and all individuals and organization(s) and their respective representatives and/or agents, waive any rights or claims for damages arising out of the enforcement of this rule. It is also understood that any and all person(s) and person(s) affiliated with suspected person(s), will also be removed from Conference, the Venue and future CMT activities and they also waive any rights or claims for damages arising out of the enforcement of this rule and have no recourse for legal action or remuneration from CMT, it’s respective agents, employees, representatives or affiliated business(s).

    Conference and any event App and social media use/post is considered a public forum and you hold harmless CMT. You understand that while CMT will do their best as Conference organizer(s), you agree this is a public forum and that various opinions may be shared which may be different than your own. It is also understood that screenshots, videos, comments from various parties, attendees and sponsor participants may be posted on social media without your knowledge. You hereby acknowledge this will take place at Conference and consent to release to have your photograph, likeness, video recording, audio, company logo(s), and the like. In which case you release CMT and individual(s) posting from any damages and/or liability because of your participation in Conference.

    The use of profanity (live or virtually) and/or spam posts, etc., while rare … may/will be requested for removal from social media but please do understand these comments, and any photographs taken with or without your knowledge, etc., may last for days, weeks, months or years in the public on online. These instances can, do and may happen in this environment and our public forum(s) both virtually and in-person, do come with that risk. You accept that risk by being here.

    Seek permission before you post. You consent that if you post to social media or take a photo, video, or audio recording, before, during or after the Conference, you will make your best effort to seek to gain the permission of the person(s) before posting. Furthermore, you and all the parties mentioned or in post agree to indemnify CMT, it’s respective agents, representatives and CMT affiliated companies and representatives from any claims for damages arising out of participating in this Conference.

    Be respectful online. CMT expects every person(s), exhibitor, sponsor, attendee, physician, spouse, representative, staff, and person attending Conference to be respectful in what they write, post, photograph, record, or comment on or say about others. You agree to be respectful online and will promise to treat attendees, speakers, sponsors, exhibitors, colleagues, competitors, etc., with the utmost respect and in the highest regard, both professionally and personally.

  • Refund Policy. All Conference requests for refund and/or cancellations received on or before August 19th, 20— at 11:59pm ET will receive a 50% refund. Conference cancellation requests must be submitted in writing. No refunds will be issued after August 19th, 20— at 11:59pm ET. Rollover Requests (see below) are permitted. Substitutions and Transfers are permitted for an additional fee of $25 when submitted in writing only by October 1st, 20— at 11:59pm ET. Please email all written cancellation requests to editor@conciergemedicinetoday.org.

    No Shows. Refunds are not given for no-shows.

    Substitutions and Transfers. If you are unable to attend Conference, you may transfer your registration for $25 to another person by requesting CMT change the name of your registration by email to editor@conciergemedicinetoday.org by October 1st, 20—, at 11:59 pm EST. Acceptance of Registration Terms and Conditions will automatically transfer/apply to the recipient(s). Please note: (1) The substituted individual must be from the same organization as the original registrant; (2) A maximum of one (1) substitution per original registrant is allowed; and (3) There will be a $25 processing fee for any substitution required.

    Rollover Requests. If you are unable to attend Conference for any reason, you may request to rollover your registration at no additional cost to the next calendar year Conference by notifying CMT by email to editor@conciergemedicinetoday.org by October 1st, 20—, at 11:59 pm EST. Acceptance of Registration Terms and Conditions automatically transfers as well to the registered ticket holder. Once a rollover request has been accepted by CMT toward a future Conference, no further changes, refund requests and/or edits will be allowed. In the event of cancellation on the attendees’ behalf, the rollover request will no longer be valid and the money will not be refunded.

    Important Reminder. You are solely responsible for canceling your own hotel and travel reservations. Please remember that canceling or changing your registration will not automatically cancel any hotel and travel arrangements you made. CMT takes no responsibility for any damages, costs, or losses should you cancel or change your registration or if Conference is cancelled or postponed.

  • Under no circumstances will CMT, it’s respective agents, directors, employees, contractors, sub-contractors, affiliated businesses, partners or staff be responsible for any loss or damage, theft, including any loss or damage, personal injury, allergic reaction, food reaction, food allergy, skin issues, etc., and/or death in injury resulting from anyone’s use of our conference, catering, website(s), apps and/or Services or services provided by sponsored exhibitors, or any interactions between our contracted representatives, sponsor/vendor exhibitors, authorized representatives, etc., users of our website(s), app(s) and/or services, whether in-person, online or offline.   • Professional and personal photographs, audio, and video will be captured during the Conference. You, and anyone in your party, hereby grant CMT and its representatives’ permission to photograph and/or record you and them at the Conference and distribute (both now and in the future) the image(s) or voice in photographs, videotapes, electronic reproductions, and audiotapes of such events and activities and you consent and understand that you have waived any rights or claims for damages, remuneration, defamation, legal action, etc., arising out of the enforcement of this rule. I DO HEREBY RELEASE AND FOREVER DISCHARGE THE RELEASED PARTIES FROM ANY CLAIM WHATSOEVER WHICH ARISES OR MAY HEREAFTER ARISE ON ACCOUNT OF ANY FIRST AID, TREATMENT, OR SERVICE RENDERED IN CONNECTION WITH MY PARTICIPATION IN THE ACTIVITY.   • Please be advised that food prepared here may contain these Ingredients: Milk, Eggs, Wheat, Soybean, Peanuts, Tree Nuts, Fish, Dairy Products, and Shellfish. Please note, a limited number of meals will be available for those with dietary restrictions but should you desire to bring your own meal with you, our conference facility does allow that. You will not be refunded a portion or in-part or whole for these personal expenses. We simply ask that you do not share your meal with others.

  • You should not consume alcohol if you are pregnant or nursing or if your Physician notes that you may have an allergic reaction or drug prescription reaction. If permitted by CMT, there may be a cocktail reception. This event is TBA at a later date. However, You may not bring your own, nor leave the Venue nor the exhibit hall with alcohol. Security may be on site and authorized representatives may restrict or remove you if necessary due to alcohol consumption or behavior they deem inappropriate. Additionally, CMT contracted “pourer” arranged through caterer for any cocktail reception(s) or events may restrict your consumption of alcohol and reserves the right to refuse at any time your consumption of and admittance to the Conference for any reason without notice, refund or legal recourse. You and any and all heirs, family members, companies and/or co-workers indemnify, release and hold harmless CMT, it’s respective agents, partners, employees, representatives, contractors and sub-contractors from any liability, damages or claims related to the consumption of alcohol during a Conference activity that results in injury, incapacity, falls, slips, accidents, injury, motor vehicle accident, rideshare accident(s), heart attack, stroke, allergies, death and or the misconduct of others related to the consumption of alcohol.   • Consent to and permission to anyone around you the right to take photography, audio, and video recordings. When you enter Conference, a room, hallway, lobby, session, exhibitor hall, breakout, outdoor property, parking lot, picnic tables on property, workshops, reception, meeting, general session, etc., you consent to and permit to anyone around you the right to take photography, audio, and video recordings and understand this may occur. By entering the Conference, Venue property, premises, whether it be outside or inside the building, you understand that wherever people are gathering or people is where photographs, filming and recording is occurring. You consent to be interview(ed), get photographed, be audio recorded, video recorded, albeit in the background as well, and release, hold harmless and indemnify anyone conducting these activities for life without legal action for remuneration, losses, defamation, tortious interference, claims, financial loss, character assassinations and the like. You understand you may not record audio or video of physical or virtual sessions at any event, session, breakout or gathering, whether informal or formal while on site at the event without the express written permission of CMT.

    Future Use Permitted: Images, photos and/or videos may be used to promote similar related CMT events in the future, highlight event and exhibit the capabilities of CMT and affiliated brands or businesses. You release CMT, its officers and employees, and each and any and all persons involved from any liability connected with the using, selling, taking, recording, digitizing, marketing, promoting, emailing, printing, or publication and use of interviews, photographs, likeness, edits, computer images, video and/or or sound recordings for life.

    Waiver and Release: Furthermore, by entering the event premises, you waive and release all rights you may have to any claims for payment or royalties in connection with any reselling, use, exhibition, streaming, web casting, televising, or other publication of these materials, regardless of the purpose or sponsoring of such use, exhibiting, broadcasting, web casting, or other publication irrespective of whether a fee for admission or sponsorship is charged.

    Inspection Waiver: You also waive any right to inspect or approve any photo, video, or audio recording taken by CMT or the person or entity designated to do so by CMT.

    Fully informed: You have been fully informed of your consent, waiver of liability, and release before entering the Conference.   • Please uphold the spirit of the Code of Conduct as detailed in Concierge Medicine Today’s “Conference Participant Agreement: Terms & Conditions” (before, during, and after) our events. This includes not only the clear DOs and DON’Ts listed below and available at https://www.conciergemedicinetoday.net/faqs-cmt-events, but also a commitment to creating a safe, productive, and harassment-free environment for all participants at Concierge Medicine Today’s “Concierge Medicine Forum” (CMF).   • The registration fee includes access to conference keynotes, access to all breakouts, exhibit hall access, educational sessions, and conference materials. It also includes conference meals such as CMT-hosted continental breakfast on Saturday (8am-9am,ET) a Friday lunch buffet, refreshment breaks, and maybe, a Friday evening cocktail hour (if scheduled and/or TBD).

  • No. All you need to do is plan your transportation and hotel logistics. On either Thursday or Friday, when you arrive at the conference center, you will receive your ‘Conference Lanyard/Badge’ and at that time you must check-in with our registration table in the Forsyth Conference Center Atrium/Foyer/Lobby in order to confirm your arrival, receive your badge, additional SWAG and confirm/verify any additional name changes in your group/party. Entry is not permitted without registration and your badge.

  • You may be asked for a government id or other form of identification. Additionally, please have your “payment confirmation” or email handy at check-in. If you cannot find it, left it at the hotel or forgot it, that’s okay. As long as you registered as an Attendee, Speaker or Sponsor, we should be able to locate your name on our guest list. Please note, we may ask you to scan a QR code or sign at check-in that you have read and understand our conference participant/attendee updated terms and conditions before entry is permitted.

  • We’ve got you taken care of here! You’ll be pleased to know that there are two hotels within a short 5-10-minute walking distance from the Lanier @ Forsyth Conference Center. Additionally, there are plenty of hotel and nearby dining options. Click here for a list of 6 local hotels, 2 of which are within a short 10-20 minute walk of the conference center (depending all on how fast you walk, of course).

    Please be aware, plan for inclement weather and dress in layers. The Atlanta area during October has been known to be rainy and cold but on the other hand, hot, humid, rainy and steamy.

    Please be aware, there are no shuttles to the conference center from any hotel, so plan your transportation or ride share accordingly and plan early!

    The largest and closest international and domestic airport is as you probably already know … Hartsfield-Jackson Atlanta International Airport (Airport Code ATL).

    Rideshare, Car Rental or Taxi has been used by Conference attendees in the past. But, plan ahead as rideshare availability around the conference center and area can fill up quickly.

    Hotel options within a 2-3 exit drive or even walking distance are plentiful. Here are six local hotel options that Conference attendees have stayed at in the past!

  • Eat your heart out in Georgia, where restaurant and dining options range from casual to fine dining, and you’ll find all types of cuisines — especially Southern.

    So you have three local area options.

    First, our CMT Forum is within walking distance of two hotels, a shopping center area, multiple restaurants of all kinds, groceries, casual and fine dining, fast food and more. It’s called The Collection at Forsyth and is one option for an outdoor lifestyle shopping destination in Cumming, Ga. The Collection at Forsyth features a variety of premier national retailers, restaurants and a movie theater, as well as specialty local stores and eateries.

    Second, The Avalon. It’s about 2-3 exits away (or maybe 15-20 minutes with rideshare). It has a lot of great green space and a hotel on the property. Visit: https://www.experienceavalon.com/

  • Parking is free and available at the hotel if you are a traveling guest and is also free at the conference venue.

  • Hey, life happens to all of us. Emergencies occur and sometimes our speakers miss their flight, travel plans are delayed or they have to cancel due to unexpected circumstances. It does happens and on seldom occasions to our Conference program too. Not often, but we roll with the punches. Please note that we are unable to offer refunds due to speaker or session changes or cancellations.

  • No bags, luggage or property should be left unattended at any time. You may bring them but please understand that CMT, our team members and Venue staff are not liable or responsible for your property in any way. Unattended property may be moved or removed so if you do bring your luggage, please keep it near you at all time. CMT, its respective agents, employees, representatives are not liable for missing, lost, unattended, damaged, left behind or stolen property.   • Concierge Medicine Today, LLC is not responsible for lost, left behind, stolen or misplaced personal items, luggage, electronics, etc. We recommend you please reach out to the Forsyth Conference Center team at 678-341-6655 or email: ForsythConferenceCenter@laniertech.edu and see if anyone turned something in.

  • This is really an event you really need to experience in-person. We do not currently offer live streaming or a virtual viewing option during this Conference. However, if event organizers decide to offer this option, virtual tickets must be purchased through our event app. At present, no live stream is being planned. In the past, conference attendees have mentioned that it can distract the audiences. We may record certain sessions for playback and post them in our event app for paid ticket holders, highlighting certain speakers and sessions who provide exclusive conference content via webinar recordings. Any recordings, live streams or exclusive releases will be available for purchase in our event app and released in November, following the conclusion of our Concierge Medicine Forum, annual conference.

  • We don’t have control over this but that’s a great question, and an important one. If this is important to you and critical as to whether or not you attend a conference, please feel free to call the Facilities Director/Event Planner at the Forsyth Conference Center at (678) 341-6633.

  • No. Except as specifically pre-approved in writing by CMT, no persons under 18 years of age will be permitted at Conference, including in meal breaks, cocktail receptions, lobby meetings, general sessions, breakouts and all other Conference venues or activities.

  • At this time, we do not offer translators or translation services. Due to the disruption and discomfort this may create for the majority of our guests, no audible translations will be permitted in any breakouts or main sessions.

  • Yes! Of course we’ll have coffee! And a lot of it! There will be free, decaf, regular and hot water and tea options available throughout the conference.

  • There are strict rules against animals inside certain facilities per the Georgia State Department of Health. Any and all pets and/or service animals must be pre-approved by the Forsyth Conference Center and Concierge Medicine Today, LLC event organizers. Health and safety standards must be met, and all participants must follow the health code regarding pets in a dining establishment.

  • Good question. We’ve been doing this for a very long time. We understand the needs our attendees and our Speakers. We routinely have over 50% of our Speakers so excited about their presentation at our FORUM that they often make last minute adjustments to their presentations to you at the last minute. Therefore, it’s impossible to have all those changes inserted, printed and distributed to our guests before you arrive.
    We will however, distribute all conference materials at check-in or during the event. PLUS, we make ALL of our “Permitted” Slide Decks available within 10-business days AFTER the Conference to ALL of our Attendees. Not too mention, if you look at the reverse side of your name badge/printed lanyard, you’ll find instructions on how and where to download these “Slide Decks” which have ALL the changes we talked about inserted into them that our faculty, Physicians and presenters may have made along the way!

  • If you do not feel well or are experiencing any symptoms of COVID-19, please do not attend.

    Upon entering the event venue, attendees may be asked to scan a QR code that will take them to a questionnaire to mark their attendance, answer screening questions, and have their temperatures taken. Temperatures may be taken by a masked volunteer or employee or representative. If an in-person event attendee has a temperature of 100.4 or greater they will NOT be permitted in the event venue.

    There may be multiple sanitation stations located throughout the venue. If required, please follow all social distancing guidelines including, remaining 6 ft. apart and refrain from shaking hands. “Any person entering the premises waives all civil liability against this premises owner and operator for any injuries caused by the inherent risk associated with contracting COVID-19 at public gatherings, except for gross negligence, willful and wanton misconduct, reckless infliction of harm, or intentional infliction of harm, by the individual or entity of the premises.”

    On Friday, October 20, after you’ve checked in and received your badge, you will enter our Exhibit Hall/The Welby Room and network with friends, our wonderful exhibitors, find a seat and rest. On Friday, the conference will kick off at 8:45a.m. ET, sharp! Our last session of the day should conclude at 6pm ET.
